Terms & Conditions of Service

Please read these terms carefully before submitting your medical questionnaire.

1. Platform Nature & Medical Services

Mounjaro Germany operates a digital health platform facilitating patient access to licensed doctors and licensed dispensing pharmacies serving patients in Germany. Completing a consultation questionnaire does not guarantee that a prescription will be issued. Clinical authority to prescribe Mounjaro® rests exclusively with the independent examining prescriber.

2. Patient Eligibility & Age Requirements

To utilize this service, you must:

  • Be at least 18 years of age;
  • Be physically resident in Germany with a delivery address in Germany;
  • Provide accurate, truthful, and up-to-date medical history, including current medications, known allergies, personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2;
  • Not complete the medical questionnaire on behalf of another individual.

3. Patient Obligations & Truthful Disclosure

Declaration of Accuracy: Providing false, misleading, or incomplete clinical information during the assessment poses severe risks to your personal health. You warrant that all answers regarding current BMI, medical conditions, and lifestyle are true and accurate to the best of your knowledge.

4. Treatment Protocol & Administration

Patients agree to administer Mounjaro strictly in accordance with the Patient Information Leaflet (PIL) and specific dosing instructions provided by the prescribing clinician. Doses must never be escalated faster than the clinical titration protocol. You agree to notify our medical team or your family doctor (Hausarzt) if you experience unexpected or severe side effects.

5. Governing Law & Jurisdiction

These Terms and Conditions shall be governed by and construed in accordance with the laws of the Federal Republic of Germany. Any disputes arising in connection with this agreement shall be subject to the jurisdiction of the competent German courts, without prejudice to any mandatory consumer protection rights.
